Step 2: Admission Officer shall gather all data and secure consent forms.
Step 3: Input the gathered information to the hospital information system
Step 4: Confirm the patient’s room of choice and coordinate it to the nurse station
Step 5: The Nursing Assistant will assist the patient to their designated room.
ER Admission
Walk-in patients with no doctor’s admission order may proceed to the ER to be assessed and referred to the admitting section.
Step 1: The patient will be assess by our resident on duty or er consultant if he/she is admissible.
Step 2: The ER secretary will interview the patient’s relative for the information, and will ask if he/she has health insurance. If yes, the secretary will call the specific health insurance hotline for approval and verification.
Step 3: The ER staff/secretary will inform the patient to proceed to information & admitting office to seek or ask for a room that will be covered by his/her health insurance.
Step 4: The information & admitting staff will give the admitting slip, admission chart (admission agreement, waiver and consent forms), and HMO data sheet to be filled out by the relative/patient.
Step 5: The information & admitting staff will ask for the HMO card and philhealth ID. They will also ask the relative/patient to proceed to philhealth window 1 with the philhealth eligibility form for verification.
Step 6: After the verification, the patient/relative will return to Information & Admitting Office to complete all admission papers.
